VIVA Credentials

Credentials Service - For Veterinarians Only

The AAVSB will collect license verifications and veterinary school transcripts (or PAVE certificate or ECFVG certificate) required by the AAVSB Member Boards for veterinary licensure, and transfer it to the veterinary licensing board you request. Compiling a complete credentials packet takes 5 to 7 weeks and includes an official national licensing exam score report. Applicants must first Register for Credentials. Once an applicant is registered, they can apply for additional Credential services by completing a Credentials Update service form.


Step 1 - Complete:

Step 2 - Print, complete, sign and mail to AAVSB at the address below:


Print Affidavit for Release of Information


Step 3 - Mail an original or certified copy of your birth certificate or a notarized copy of your passport to the AAVSB.


Step 4 - If your name is different from your passport or birth certificate, mail a notorized copy of the document that authorized your name change (for example: a marriage certificate, divorce decree, or legal name-change document) to the AAVSB.


The AAVSB must receive all documents before service is initialized. Credentials request CANNOT be expedited. Mail all documents to:
