Fees and Processing Time
The cost of VAULT Premium is $190 per transfer destination, plus any outside administrative fees such as transcript or license verification fees.

Estimated Processing Time: Once the licensee has sent their notarized affidavit and supporting documents to the AAVSB, please allow 5-7 weeks
for completion of this service. State and provincial licensing boards have varying levels of staffing and processing speeds.

1: Complete the VAULT Premium online application.
Payment is due at the time of application. Once your application has been submitted with payment, there are no refunds.
Note: Arizona,Massachusetts,Missouri,Quebec, and Washington
currently may not be selected as a transfer destination in the VAULT Premium Transfer Service.
2: Submit documents to the AAVSB
Print, complete & sign, notarize, and mail the affidavit for release of information to the AAVSB.
Confirm your identity by submitting one of the following:
Original birth certificate
certified copy of your birth certificate
notarized copy of your birth certificate
notarized copy of your passport
Please do not send your original passport as the AAVSB retains all documents.
If your name is different from your passport or birth certificate, include a notarized copy of your marriage license, divorce decree, or other name-change document.
3: Mail notarized affidavit and supporting documents to the AAVSB:

Not all electronic notaries can service international documentation.
